Authorization: Camp tuition is payable in advance. It is non-refundable and no make-ups are available. As legal parent or guardian of this athlete, I hereby verify by my signature below that I fully understand and accept each of the above conditions for permitting my child to participate in competitive team gymnastics training camp at Manakoa Gymnastics. WAIVER & RELEASE: I am aware that gymnastics is a dangerous activity that could result in injury, paralysis, or even death. I assume all risks of injury or loss to myself or my child arising from the Manakoa Gymnastics' classes/programs/activities. If the participant is a minor, I also give my permission for his/her participation in the above activity, and for any necessary medical treatment. In exchange for me, and/or my child, being allowed to participate in Manakoa Gymnastics’ classes/programs/activities, I release, discharge and agree to indemnify and hold harmless Manakoa Gymnastics, its employees, officers, or agents (the “Released Parties”) from any liability, loss or damage, including but not limited to that arising from the negligence of any of the Released Parties, which may result to me or any minor child of mine. For any athlete who is not yet 18 years old: As legal parent or guardian of this athlete, I hereby verify by my signature below that I fully understand and accept each of the above conditions for permitting my child to participate in classes, events, competitions, and activities conducted at Manakoa Gymnastics.
